1. Before you fill out the following form, you must have your transcript. If you don't have your transcript ready, you can request your transcript by clicking on the following link. 

2. You must also fill out a Brag Sheet Form and attach it to the form below when prompted. You can download the Brag Sheet here.

• ESSLCE is the Ethiopian Secondary School Leaving Certificate Examination, formerly known as the EUEE (Ethiopian University Entrance Examination).

• EGSECE is the Ethiopian General Secondary Education Certificate Examination.

1. • The only accepted file type is PDF.

   • The maximum file size accepted is (for each file) 2 MB. If it is larger than 2 MB, please compress the file to make it less. 

   • You can upload up to 5 files. Generally, only one is enough if you attended St John Baptist De La Salle Catholic School throughout your high school. If not, you can upload transcripts from other institutions as well.
